2024 COMMUNITY HEALTH ASSESSMENT
The Green County Healthy Community Coalition (GCHCC) has completed the 2024 Green County Community Health Assessment. This is a process the coalition undertakes every three years to understand Green County’s strengths and areas for improvement related to health. We are asking for input from community members through a survey (thank you to those who completed the survey!), community conversations, and key informant interviews. We'll also be collecting secondary data from sources like the U.S. Census and Wisconsin Department of Health Services. This information will help us determine which health issues our coalition needs to focus on so we can develop a plan to address them.

IDENTIFYING NEEDS
An assessment process takes place over six months, which incorporates components of primary data collection and secondary data analysis that focused on the health and social needs of Green County. The assessment also includes collection and analysis of input from persons who represent the broad interests of the community.
DETERMINING PRIORITIES
In addition to our assessment data findings, GCHCC also utilizes Healthiest Wisconsin 2020 (the state public health agenda) and Healthy People 2020 (the national agenda) as blueprints to systematically identify Green County's health priorities.
The Green County health priorities, detailed in the 2024 Community Health Improvement Plan, were determined by a facilitated group consensus method and are as follows:
