FOSTERING COLLABORATION

The Green County Healthy Community Coalition (GCHCC) is co-chaired by representatives from Green County Public Health Department and Monroe Clinic. GCHCC is comprised of over 50 organizations representing local governmental agencies, community organizations, educational institutions and businesses.

 
 

IDENTIFYING NEEDS

An assessment process took place over six months, which incorporated components of primary data collection and secondary data analysis that focused on the health and social needs of Green County. The assessment also included collection and analysis of input from persons who represented the broad interests of the community.

DETERMINING PRIORITIES

In addition to our assessment data findings, GCHCC also utilized Healthiest Wisconsin 2020 (the state public health agenda) and Healthy People 2020 (the national agenda) as blueprints to systematically identify Green County's health priorities.

The Green County health priorities, detailed in the 2018 Community Health Improvement Plan, were determined by a facilitated group consensus method and are as follows:
